Lost Social Security Card:
Report lost or stolen cards to the nearest Police Station in your jurisdiction.
To Reapply:

• Apply for a Police Record
• Pay $25.00 for a replacement card
• birth certificate or passport if not already brought in

Lost  or stolen Bank Cards:
Apply for a new one, cost $15.00.
Lost or stolen cards are usually replaced within 3-5 days after you make a report. If you’re in the
US, replacement usually takes place within 48 hours.

Lost Voter’s Identification Card:
First Card Issued is free of cost.

Replacement card fee $5.00 Second replacement is $10.00.

Lost Passport:
Obtain the following:

Police Record

Original Birth Paper

Voter’s Identification Card or Social Security Card

Need to get a declaration form stating how passport was lost.

A fee of $50.00 is charged if lost, stolen or damaged.
Damaged passports require only a declaration form.

Lost Driver’s License:
Report to the District Transport Office.

• Obtain application form at a transport office in your district
• Police Record required.
